[Eratechat] Need advice on: purchase orders for equipment

I am getting ready to file 471 and want to include a Block 5 entry for a vendor that will provide equipment only.  I believe I read somewhere that I can use the Purchase Order rather than a signed contract.  Is this correct?

I am new to this and filing for VoIP, so I will probably have a lot of questions in the next several days, so please bear with me.
