[Mo-facs] Request for Your Participation in a Short (10-minute) Online Survey

Dear MO-FACS Listserv Members,


As a member of the Missouri Coordinated School Health Coalition (MCSHC), I am personally requesting your participation in a short (10-minute) online survey to help MCSHC determine which tools and resources to share with you on coordinated school health.  

 

As a bonus, if you register and complete the survey, you will be eligible for a drawing for free entrance into our annual conference to be held from Thursday, November 29, to Saturday, December 1, 2007, at the Lodge of the Four Seasons in Osage Beach, Missouri.  More information is available at http://www.healthykidsmo.org/conferences/index.htm.  The winner will be notified at the end of June.

 

To complete the survey, please go to http://www.surveymonkey.com/s.asp?u=82223830045.  We ask that you complete the survey no later than Friday, June 1, 2007.  As part of this e-mail, we have attached the list of survey questions.  

 

Individual responses will not be made available to us.  We have hired an independent consultant, Gretchen Kunkel, to collect the responses and prepare a report of the findings.  She will keep your individual responses completely confidential.
