[Mo-facs] cookbook making

I'd like to have my summer school cooking class make a cookbook to write in and use for our 5 week session. Does anyone have directions or suggestions for putting one together? I'm looking for a "pre-made" direction sheet of some-sort so I don't have to create one from scratch.
